Ventilator-associated pneumonia (VAP)

Also known as Ventilator-associated pneumonia (VAP) · Hospital-acquired pneumonia (HAP) · CPIS (Clinical Pulmonary Infection Score) · VAP prevention bundle

VAP is pneumonia developing more than 48 hours after endotracheal intubation. It is one of the most frequent ICU-acquired infections, with attributable mortality of around 10 percent. Pathogenesis: microaspiration of oropharyngeal secretions around the ETT cuff, bacterial colonisation of the ETT biofilm. Diagnosis: new/persistent infiltrate on CXR PLUS at least 2 of fever, leukocytosis, purulent secretions, with CPIS as a support tool. Microbiology: early VAP (under 5 days) — S. pneumoniae, H. influenzae, MSSA. Late VAP (5 days or more) — MRSA, Pseudomonas, Acinetobacter, ESBL. Treatment: empiric antibiotics promptly, guided by MDR risk, de-escalate when susceptibility results return. Duration: 7 days standard (8 vs 15 day trial). Prevention bundle: head elevation 30-45 degrees, daily sedation interruption, oral care, subglottic suction, cuff pressure control.

Red flags

  • Prioritise prompt, appropriate empiric antibiotics — then de-escalate as soon as susceptibility results return
  • Late VAP (>5 days, prolonged ICU stay) requires broad-spectrum cover for MDR organisms: MRSA, Pseudomonas, Acinetobacter, ESBL
  • Do NOT routinely use antibiotics for prevention (SDD controversial — selective decontamination not standard in most units)
  • Standard duration is 7 days unless S. aureus or non-fermenting Gram-negatives — then 7-8 days (8 vs 15-day trial showed equivalence)

The one-line answer

VAP is pneumonia arising more than 48 hours after endotracheal intubation — one of the most frequent ICU-acquired infections, with attributable mortality around 10 percent. Diagnose on a new or progressive CXR infiltrate plus at least two of fever, leukocytosis, or purulent secretions, with CPIS as a support tool. Split by onset: early (under 5 days) is community-type organisms; late (5 days or more, or any MDR risk factor) is MRSA, Pseudomonas, Acinetobacter, ESBL. Treat empirically without delay — MRSA cover plus an anti-pseudomonal beta-lactam — then narrow as soon as susceptibility results return. Duration is 7 days (the 8-vs-15-day trial showed equivalence). Prevent with the bundle: head up 30–45, daily sedation break, oral care, subglottic suction.[1][2]

Meet the patient

A 68-year-old man is day 9 of ventilation after a laparotomy for perforated diverticulitis, already five days into piperacillin-tazobactam. At 3am the nurse pages you: temperature 38.7°C, secretions thick and green, and a new right lower lobe patch on the chest X-ray. Two questions will decide his next hour, and they decide every VAP — is this infection, and is the organism multidrug-resistant?[1]

The clock has already started — appropriate empiric cover drives outcome, so you will draw cultures, start broad therapy promptly, and narrow everything when the plates return.[1]
