Meet the patient
A 39-year-old man, two years into an asylum claim, is referred by his GP for headaches and poor sleep. He was held in political detention abroad and beaten. He now lives in shared housing on a temporary visa, cannot work, and has not seen his children since he left. Every brown government envelope makes his chest tight. He tells you, through an interpreter, that he is "still in the cell when I sleep".[2][4]
The examiner's question is not "does he have PTSD?" — it is "what is making him unwell now, and what would you treat first?" The honest answer spans a war he survived, a cell he cannot forget, and a visa letter that re-arms both every month.[2][12]
