Meet the patient
A 28-year-old receptionist is in front of you for the third time this year. Her boyfriend of six weeks paused for two seconds before replying to a text, and in the six hours since she has cut her forearm, sent forty-seven messages, and drunk half a bottle of vodka. She is certain — absolutely certain — he is about to leave her. "I know what he's thinking. He hates me."[7][6]
Every registrar reaches for the crisis protocol. The examiner's question is narrower and harder: why is she so sure of his mind, and what treatment actually rebuilds her capacity to wonder? Everything below answers that.[5][7]
