Red flags

  • Suicidal collapse after public shame, status loss, or narcissistic injury — assess intent, means, and depression under the arrogant exterior
  • Intimate partner violence, coercive control, child safeguarding, or exploitation of dependent others — legal duties are clinical duties
  • Late-onset 'personality change' with cognitive, neurological, or systemic red flags — investigate organic disease before locking a Cluster B label
  • Countertransference enactment (special treatment, competitive humiliation, sexual boundary drift, punitive discharge) — use supervision and team frame
  • Missed mania/hypomania, substance intoxication, major depression, or PTSD while attributing everything to 'narcissism'
  • Unreviewed polypharmacy presented as treatment of NPD/HPD/ASPD — no drug treats these disorders as whole diseases

The one-line answer

Cluster B collects four dramatic-erratic patterns; narcissistic personality disorder is the one built on self-esteem regulated from outside — and pathological narcissism has a grandiose and a vulnerable pole in the same person. Diagnose by longitudinal pattern and primary relational motive, treat comorbidity properly, hold a frame, read your own countertransference as data, and remember the exam's real answer: the injury is the risk event. No drug treats narcissistic, histrionic or antisocial personality disorder as a whole.[3][4][8][12]

Meet the patient

A 52-year-old surgeon is admitted after an overdose. Yesterday he was stood down pending an investigation; last night a colleague sent him the news article. On the ward he is impeccable, faintly amused, and tells you the referral was unnecessary and that he would prefer to speak to "someone more senior."[8][3]

The registrar's note reads: "declines engagement, no active suicidal ideation, for discharge." The consultant's question is the one that matters: "You have a man who took an overdose twelve hours after public humiliation. What made you confident the risk had passed?"[8][5]

Arrogance is not a protective factor. It is a presentation. This topic is about not letting the presentation do your risk assessment for you.[8][5]
