A 17-year-old with first-episode psychosis is two months stable on a long-acting antipsychotic when CAMHS sends a referral that reads "approaching upper age limit — please accept for adult services". There is no attached formulation, no risk summary, no named clinician, and her mother tells you she has not been told this is happening. Adult triage will take six weeks. Her eighteenth birthday is in three.[2][6]
Two questions are live. Who owns this young person between now and her first adult appointment? And what exactly is being transferred — a person, or a file? The TRACK answer to both is the same uncomfortable one: most of the time, nobody, and a file.[2][7]
