Meet the patient
A four-year-old arrives in ED with a spiral fracture of the humerus. The stated history is a roll off the couch. Her mother answers every question; the child says nothing and looks at her mother before each answer. The record shows two previous injury presentations at two other hospitals in eight months.[3][13]
You are the psychiatry registrar, asked to see the mother — who has a documented depressive illness, is distressed, and wants to take her daughter home tonight.[3][17]
