The answer first

The one-line answer

Undifferentiated abdominal pain sorts into two bedside questions: is the patient sick or well? and is this peritonism or colic? — resuscitate the sick, image with CT abdomen and pelvis with IV contrast, and refer surgically for peritonism or a surgical cause. The must-not-miss list is vascular and lethal: ruptured AAA, mesenteric ischaemia, perforated viscus, ectopic pregnancy, aortic dissection, DKA, sickle cell crisis, and adrenal crisis in the patient on chronic steroids.[1]
