A seven-year-old with bilateral spastic cerebral palsy rolls into clinic in a wheelchair. The parent asks whether the hips are “still OK,” whether botulinum toxin is due, and why school says fine-motor work is falling behind. Your job today is not to re-diagnose CP from scratch. Your job is surveillance and multidisciplinary management: functional classification, prevention of secondary harm, coordinated therapy, and a plan the family can use. [1] [18]
The consensus definition still anchors the viva. Cerebral palsy describes permanent disorders of the development of movement and posture, causing activity limitation, that are attributed to non-progressive disturbances in the developing fetal or infant brain. The motor disorder is often accompanied by disturbances of sensation, perception, cognition, communication and behaviour, by epilepsy, and by secondary musculoskeletal problems. [1]
That last clause is the surveillance engine. The brain injury does not “progress like a tumour,” but the hips, spine, nutrition, airways and participation can deteriorate if nobody is watching.
