A preterm infant on the ventilator suddenly desaturates, bradys, and the ventilator pressures climb. You have minutes. Neonatal air-leak syndromes are a family of conditions in which air escapes the lung's gas-exchanging spaces through a tear in the alveolus or terminal airway and accumulates somewhere it should not. They are not five separate diseases but five destinations of one pathological event: alveolar over-distension rupturing the wall of a terminal air sac. [3] [8]
The unifying mechanism is pressure. When the transalveolar pressure — the pressure gradient across the alveolar wall — exceeds the tensile strength of the terminal air sac, the wall tears and air is forced into the interstitium. From there it can track along the peribronchovascular connective tissue sheaths toward the hilum (the Macklin effect), reaching the mediastinum, pericardium, peritoneum, or rupture through the visceral pleura into the pleural space. One rupture can therefore produce several syndromes at once, and a single site of collection can be life-threatening if it is under tension. [3] [12]
The clinical importance of an air leak is governed by two questions: where is the air? and is it under tension? A small pneumomediastinum in a stable term infant may be an incidental radiograph finding needing no treatment. A tension pneumothorax or a pneumopericardium with tamponade produces cardiovascular collapse within minutes. Recognising the difference, and knowing when to act before imaging, is the heart of this topic. [1] [9]
The modern story of air leaks is largely one of prevention working. Antenatal corticosteroids, prophylactic and early surfactant, and gentler synchronised volume-targeted ventilation have steadily reduced the incidence over three decades — a trend the fellowship candidate must credit, because it is the evidence base for the prevention bundle you will be asked to recite.
