Red flags

  • Voiding-dysfunction symptoms before SUI surgery — NICE indication for multichannel cystometry
  • Previous SUI surgery with recurrent leakage — perform cystometry before further surgery
  • Anterior or apical prolapse with stress leakage — cystometry indicated; occult SUI found in 29.4% evaluated
  • Urge-predominant or unclear-type incontinence — cystometry before any SUI surgery

One-line fellowship answer

Urodynamics measures lower urinary tract function by uroflowmetry, cystometry and pressure-flow analysis to ICS standards; it does not improve outcomes before primary stress-incontinence surgery when SUI is clinically demonstrated (NICE: do not perform it), it is indicated when the picture is mixed or unclear, with voiding symptoms, anterior or apical prolapse or previous SUI surgery, and in refractory overactive bladder the FUTURE trial found it neither superior nor cost-effective.

A 47-year-old woman with stress leakage on coughing and a positive cough stress test asks whether she needs "the bladder pressure test" before a sling. The answer depends on whether stress incontinence is clinically demonstrated and on whether any of NICE's four indications for preoperative cystometry are present.[1]

What urodynamics is

The ICS Good Urodynamic Practices and Terms 2016 updated the 2002 standard: it newly or more precisely defined more than 30 terms and set standards for the practice, quality control, interpretation and reporting of urodynamics, cystometry and pressure-flow analysis, with recommendations for pre-test information and patient preparation.[2] The joint IUGA/ICS terminology report for female pelvic floor dysfunction contains more than 250 definitions and defines the six most common diagnoses.[7]

The component studies
StudyWhat it measuresKey points
UroflowmetryUrine flow over time during micturitionNon-invasive screen for voiding difficulty; maximum and average flow rates and voided volume matter; flow depends strongly on voided volume, best corrected with the Liverpool nomograms
Filling cystometryBladder pressure-volume relationship during fillingPart of multichannel filling and voiding cystometry; standards set by ICS-GUP2016
Pressure-flow studyDetrusor pressure and flow during voidingPressure-flow analysis standardised by ICS-GUP2016
