Red flags
- Visible haematuria with incontinence is a urological referral, not a continence assessment — bladder cancer must be excluded first
- New incontinence with saddle numbness, reduced anal tone or bilateral leg signs suggests cauda equina syndrome — emergency imaging, not pelvic floor exercises
- Incontinence after pelvic surgery or obstetric trauma with continuous leakage suggests a fistula — refer for specialist assessment the same month
- A hot, painful, swollen calf with new incontinence is not the bladder — think pelvic malignancy or retroperitoneal disease compressing nerves, and examine properly
- New urinary incontinence with cognitive decline in an older woman on multiple anticholinergics — the treatment may be the cause; review the medication list before escalating therapy
Overview
Urinary incontinence is any involuntary leakage of urine. It is a symptom, not a diagnosis — the diagnosis is the mechanism behind it. The EPIC population survey across five countries found 64.3% of adults reported at least one lower urinary tract symptom, with overall urinary incontinence at 9.4% and overactive bladder at 11.8%.[2] Worldwide modelling projects 423 million people with urinary incontinence, a 21.6% increase over a decade.[5]
The defining general practice statistic is not prevalence but silence. In the Norwegian EPINCONT study, only 26% of incontinent women had ever seen a doctor about it; even among women with significant, troublesome incontinence, only half had consulted.[1] Among young women aged 22–27, 11% reported incontinence in the past month, and only one in five had sought help.[9] The condition also costs working women directly: symptom severity predicts workplace productivity impairment, yet 60% of affected working women had never contacted a doctor and 72.7% had never been treated.[23]
