A 54-year-old office manager attends for a routine blood pressure review. Her new-patient form says 'bladder trouble — don't want to discuss'. She is overweight (BMI 33), menopausal, not on hormone therapy. When you ask gently, she admits leaking 'a bit' when she sneezes and 'sometimes on the way to the toilet'. She uses pads daily, has told no one, and says her mother 'had that mesh surgery and it was a disaster'. She wants to know if anything can even be done.
Objectives
- Open the disclosure safely and characterise both components of her leakage. [1]
- Complete the core assessment: examination including atrophy and pelvic floor contraction, urinalysis, bladder diary. [21]
- Deliver the mixed-incontinence diagnosis and start the conservative ladder with evidence. [18] [3] [22]
- Address the mother's mesh story with honest, quantified surgical information. [19] [24]
- Agree a review plan that keeps her engaged. [26]
