Red flags
- Fever, rigors or flank pain change the diagnosis from cystitis to pyelonephritis — different drug, different duration, different follow-up; a woman treated for 'cystitis' who returns vomiting is being re-triaged, not re-prescribed
- Bacteriuria without symptoms is not an infection: treating it in catheters or older adults breeds resistance and does not touch delirium — systematic review found no effect of antibiotics on delirium outcomes in 652 older adults with pyuria or bacteriuria
- A man with a UTI needs culture and review of the whole tract — first infections may unmask prostatitis, stones or malignancy; do not treat him like a 22-year-old woman with dysuria
- Visible haematuria after treatment resolves needs urological referral on its own merits, whatever the cultures show
Overview
Urinary tract infection spans two diseases wearing one name. The first — acute uncomplicated cystitis of healthy women — is common, self-limiting in many, diagnosable from history alone, and cured by short courses of narrow-spectrum antibiotics. The second — infection complicated by male anatomy, pregnancy, abnormal tract, stone, catheter or kidney involvement — demands cultures, longer or parenteral therapy, and a search for what made the host vulnerable. Confusing them drives both under-treatment and antibiotic overuse.[5]
The organism is usually the patient's own bowel flora: uropathogenic Escherichia coli carrying fimbrial adhesins such as FimH and PapG climbs from perineum to bladder, binds urothelium, invades superficial cells and can form intracellular biofilm-like reservoirs that plausibly seed recurrence.[19][20] Recurrence risk may also travel through the gut-bladder axis: women with recurrent UTI show gut microbiome depletion distinct from controls.[18]
