Recurrent UTI asking for 'something stronger' — CCE clinical encounter
CCE-style clinical encounter: a 52-year-old postmenopausal woman with her fourth culture-proven cystitis this year requesting 'stronger antibiotics'. The candidate must confirm the recurrence pattern, resist escalation, sequence antibiotic-sparing prophylaxis with honest evidence, and safety-net.

Prompt
A 52-year-old postmenopausal office manager attends for 'the strongest antibiotic you've got — the usual ones aren't working anymore'. Four culture-proven E. coli cystitis episodes in twelve months, all fully sensitive. She drinks two coffees and one glass of water daily. No diabetes. No haematuria. Vaginal dryness since menopause, untreated.
Objectives
- Reframe 'stronger antibiotics' as a recurrence-management consultation without dismissing her frustration. [11]
- Confirm the recurrence definition and review the cultures for true resistance versus sensitive-organism repetition. [11]
- Sequence antibiotic-sparing prophylaxis with quantified evidence — water first, then methenamine hippurate, vaginal oestrogen, cranberry. [13] [10] [12] [14]
- Address the postmenopausal genital tract — atrophy is a modifiable driver. [12]
- Set the fallback (continuous or post-coital prophylaxis) and safety-net for pyelonephritis and haematuria. [13] [4]
