A 49-year-old office manager attends for a script refill but mentions, almost in passing, 'and I've been constipated forever'. Two motions weekly with hard stools and straining; uses herbal tablets irregularly. No bleeding that she has noticed. Father had bowel polyps. She finds the topic mortifying and keeps apologising.
Objectives
- Normalise the consultation so a full bowel history becomes possible. [10]
- Take structured history: frequency, Bristol type, straining, incompleteness, manual manoeuvres, medications. [10]
- Screen alarm features explicitly given father's polyp history. [9]
- Examine including DRE with informed consent. [12]
- Prescribe the foundation (diet + osmotic) with realistic timelines and follow-up. [9] [5]
