CCE-style clinical encounter: a mother at her baby's 4-month check is frightened by an anti-vaccine post about autism and immune overload. A 27-year-old mother attends with her 4-month-old for scheduled vaccines but hesitates at the door: 'I saw a post saying the MMR causes autism — I'm not doing that one today. And she has a bit of a snuffle anyway, so maybe we should wait.' The baby is well: alert, afebrile, mild clear rhinorrhoea. First two vaccines completed on time; no previous concerns.
Objectives
- Use presumptive framing to open the vaccination conversation rather than asking permission. [10]
- Answer the MMR-autism fear with the Danish cohort data (HR 0.93) and name the myth's fraudulent origin without shaming. [1] [3]
- Refute immune overload with the antigen-exposure case-control data (aOR ≈ 1.00). [5]
- Correct the false contraindication — mild coryza does not defer vaccines. [4]
- Note MMR is not even due today (12 months) — decouple her fear from today's needles, vaccinate, and record in AIR.
