Data viva — the GABA-A receptor and the benzodiazepine binding site

A data viva on the GABA-A receptor diagram — the three binding sites; the benzodiazepine at the alpha-gamma interface increasing the FREQUENCY (more frequent shorter bursts) versus the barbiturate increasing the DURATION (fewer longer bursts); the ceiling effect (the benzodiazepine cannot open the channel directly); the flumazenil as the competitive antagonist (displaces, no intrinsic effect); the clinical translation to the midazolam, the diazepam, the lorazepam and the remimazolam; and the remimazolam as the ester-hydrolysed ultra-short agent with the constant context-sensitive half-time.

Prompt
A schematic of the GABA-A pentameric chloride channel showing three binding sites: GABA at the orthosteric site, the benzodiazepine at the alpha-gamma interface (trace shows increased FREQUENCY of channel opening), the barbiturate at a separate allosteric site (trace shows increased DURATION), and flumazenil competing at the benzodiazepine site. Interpret the diagram and discuss the benzodiazepines.

Stimulus

A schematic of the GABA-A pentameric chloride channel is shown. It has three labelled binding sites. GABA binds at the orthosteric site (between the beta and the alpha subunits) and opens the chloride channel. The benzodiazepine binds at the alpha-gamma interface; an inset trace shows the channel opening more frequently in shorter bursts (the increased FREQUENCY). The barbiturate binds at a separate allosteric site; an inset trace shows the channel opening less often but for longer (the increased DURATION). Flumazenil is drawn competing at the benzodiazepine (alpha-gamma) site. Interpret the diagram and discuss the benzodiazepines in anaesthesia.
