Design multimodal analgesic plans for (a) day-case inguinal hernia repair in a 2-year-old and (b) tonsillectomy in a 5-year-old with OSA risk. Include pain assessment tools, exact drug doses, monitoring, and drugs you will not prescribe.
Model answer
Assessment. FLACC for non-verbal; faces/numeric when able.[1]
Hernia. Paracetamol 15 mg/kg; ibuprofen 5–10 mg/kg if appropriate; caudal or infiltration; minimal opioid; day-case education, per local paediatric-analgesia protocol.[1]
Tonsillectomy/OSA. Multimodal non-opioids; local technique; careful opioid if needed with extended observation; no codeine (CYP2D6).[2][3]
