- Acute suicidality or high-lethality self-harm after rejection, outing, violence, or discrimination — emergency psychiatric pathway with affirming communication
- Conversion or coercive efforts to change gender identity — associated with harm; do not recommend or collude
- Forced outing, intimate partner violence, or homelessness after family/partner rejection — safeguarding and multiagency safety response
- Healthcare trauma driving avoidance of essential medical/psychiatric care — rebuild trust; do not re-traumatise with deadnaming or curiosity questions
- Active psychosis or mania impairing capacity when irreversible medical decisions are also requested — stabilise mental state first
- Child or vulnerable adult at risk of abuse or exploitation — jurisdiction-specific mandatory reporting duties apply alongside affirming care
Meet the patient
A 29-year-old non-binary patient presents to the emergency department overnight after an overdose. They have a long history of depression. In the last month they were outed at work, misgendered repeatedly by a new manager, and lost their share-house when a housemate became hostile. They have not seen a GP in four years — "the last one asked me what was between my legs before asking why I came". They are not asking for hormones. They are asking to be safe tonight.[7][14][15]
This is the leaf for the patient who may never meet gender-dysphoria criteria, who may want only social or legal affirmation, or who presents primarily with trauma, discrimination, or healthcare avoidance. The examiner is testing systems, ethics, stigma and population mental health — not the diagnostic pathway, which lives in the sibling topic.[4][5][11]
