- Completing a high-risk tele-assessment without confirming patient location, emergency contacts, and local backup
- Prescribing clozapine, lithium, or other high-monitoring agents without a workable remote monitoring pathway
- Assuming low service contact means low community prevalence or low suicide risk
- Token telehealth without cultural safety, interpreter access, or primary-care integration
- Dual relationships and confidentiality breaches in small communities left unmanaged
- Delaying medical retrieval for NMS, serotonin toxicity, lithium toxicity, or medically unstable psychosis
Meet the patient
A 54-year-old sheep farmer, four hours from the nearest psychiatry clinic, presents to his rural GP during the third year of drought. He is sleeping badly, drinking nightly, and has told his wife the farm "would be better off without him". There are two firearms in the shed. He last saw a doctor eighteen months ago, for his blood pressure.[6][10]
Three exam questions land at once: how do you assess this safely at distance, what is the system that actually reaches him, and what do you do about the firearms. This topic is the answer to all three — and the firearms are not a footnote.[10][21]
