Red flags
- Coarse tremor, ataxia, confusion or seizures on lithium — stop drug, serial levels, volume support, escalate toward EXTRIP when criteria met
- New benzodiazepine in decompensated cirrhosis with rising confusion or asterixis — treat as possible precipitated hepatic encephalopathy
- Jaundice, dark urine, coagulopathy or rising ALT/AST after a high-risk psychotropic — stop culprit and involve hepatology (DILI pathway)
- Valproate with unexplained lethargy or delirium and high ammonia even if LFTs near normal — stop VPA and treat as hyperammonemic encephalopathy until proven otherwise
- Full-dose paliperidone or amisulpride in moderate–severe CKD without CrCl adjustment — accumulation and EPS risk
- AKI or dehydration on ACE inhibitor/ARB + thiazide + NSAID + lithium — chronic accumulation toxicity
Meet the patient
A 58-year-old with bipolar I, twenty years well on lithium, reaches ED with three days of confusion and a coarse tremor. She started ibuprofen for her back last week; her GP had just doubled her perindopril. Her eGFR has slid from 45 to 22. The lithium level is pending.[7][11]
A 64-year-old with decompensated alcohol-related cirrhosis and ascites was started on diazepam 5 mg three times daily for 'agitation' four days ago. He is now drowsy, asterixis positive, and his daughter says he asked her where the bathroom was in his own kitchen.[16]
Two patients, two organs, one shared question: what did the prescriber forget to do? Everything below answers that at consultant depth.[1][13]
