Treatment-resistant depression

Meet the patient

A 34-year-old teacher hands you a referral letter that says "failed five antidepressants." You read the list. Two were stopped in week two for nausea. One was sertraline 25 mg for a month. One she never collected from the pharmacy. One worked — until she stopped it herself during a good summer.[2][6]

She is still depressed, still teaching, and quietly rehearsing what she would do if it got worse. She has had zero adequate trials and one very live risk problem.[1][19]

Every registrar reaches for the sixth drug. Every examiner asks the same first question: "Is this actually treatment resistance?" This topic is that question, answered in order.[6][19]
