Paranoid personality disorder

Meet the patient

A 46-year-old man is referred by his GP after four years of workplace grievances, two employment tribunals and a formal complaint about the GP. He arrives twelve minutes early, declines to sit with his back to the door, and asks whether you record consultations. He does not want to talk about personality. He wants to talk about his sleep.[1][3]

Everything you have been taught about "engagement" is now on trial. Challenge the grievance and he is gone by minute six. Agree with it and you have joined the complaint. The examiner's question: what do you actually say first?[1][4]
