Meet the patient

A 28-year-old woman is brought to the emergency department at 2 a.m. by a domestic-violence crisis worker. She describes a coercive partner of four years, a rape the previous night, and a three-year-old asleep at home. She is trembling, dissociating in waves, and apologising for "wasting your time." She already meets criteria for acute stress disorder. The real exam questions are not the diagnosis: "What do you do in the next hour, and how do you avoid making her worse?"^[2]^[7]^[9]
