Red flags

  • Chest pain, new dyspnoea or syncope with injury attributed to long COVID without excluding pulmonary embolism, myocarditis and arrhythmia
  • New focal neurology, first seizure, fever, or unexplained weight loss labelled post-viral — investigate as new disease
  • Prescribing fixed incremental exercise escalation to a patient with post-exertional malaise
  • Writing 'no organic cause found' when the truth is that no diagnostic test for the condition exists
  • Missing suicidality in a patient who has lost work, role and income and has been disbelieved by clinicians
  • Attributing every symptom to long COVID and missing treatable sleep apnoea, anaemia, thyroid disease or a depressive episode
  • Offering unvalidated treatments (therapeutic anticoagulation, apheresis, hyperbaric oxygen) outside a trial on the strength of a research mechanism

The one-line answer

Long COVID is a fluctuating, multi-system, post-viral condition — fatigue, post-exertional malaise, cognitive dysfunction, mood and anxiety symptoms, disrupted sleep and dysautonomia persisting months after infection. Your job as the psychiatrist is not to win the mechanism argument: it is to believe the pattern, exclude the treatable, protect the energy envelope, treat comorbidity properly, and write the disability letter that works.[1][4][5][17]

Meet the patient

A 38-year-old intensive care nurse, fourteen months after an infection she describes as "a bad two weeks, nothing dramatic". She can no longer finish a handover without losing the thread. On a good day she walks four hundred metres; she then spends three days in a darkened room paying for it.[4][17]

Her bloods are normal. Two letters in her file say "anxiety". She arrives at your clinic with a plastic folder and the flat, guarded voice of someone who has rehearsed being disbelieved.[20][14]

The examiner's question is never "is it real?" It is: "What are the first three things you do in this consultation, and what would make you wrong?"[4][14]
