Heart Failure

The one-line answer

Heart failure is a clinical syndrome of breathlessness, fatigue and fluid retention driven by a structural or functional cardiac abnormality — and for HFrEF the mandate is absolute: every patient gets all four pillars of guideline-directed medical therapy, initiated together and uptitrated fast. An ARNI (or ACE inhibitor), an evidence-based beta-blocker, an MRA, and an SGLT2 inhibitor — each independently reduces mortality and hospitalisation. [1]

Meet the patient

A 72-year-old woman presents with three months of exertional dyspnoea, orthopnoea and ankle swelling. She has a background of ischaemic heart disease, type 2 diabetes, hypertension and stage 3B chronic kidney disease. The echo shows an LVEF of 32 per cent. She is on ramipril and frusemide only. [1]

The first question the long case asks is not what is wrong — the echo answered that — but what is missing from her prescription. She needs the other three pillars on board, iron studies checked, and an anticoagulation decision for her paroxysmal AF. The four-pillar framework turns a complicated patient into a checklist. [1]
