Neonatal hypothermia and thermoregulation — branching viva

Prompt
A 28-week, 950 g infant arrives in the neonatal unit at 35.0 C, pale and grunting, with a bedside glucose of 1.6 mmol/L. The examiner asks: 'Why is this infant cold, how do you classify it, and what is your management?'
Opening answer to give if asked to summarise

Neonatal hypothermia is an axillary temperature below 36.5 C, graded by WHO into mild (36.0–36.4 C), moderate (32.0–35.9 C) and severe (below 32 C). [9] The newborn loses heat faster than it can generate it through a high surface-area-to-mass ratio, thin wet skin, finite brown-fat and glycogen store, and non-shivering thermogenesis only — the preterm is the extreme. [3] In this infant, 35.0 C is moderate hypothermia, and the glucose of 1.6 mmol/L shows the metabolic cost already underway. My management is active external rewarming under a radiant warmer at roughly 0.5 C per hour, immediate glucose correction, and prevention of recurrence through the warm chain and, for the very preterm, plastic wrap. [9] [1]

Branch 1 — Why the newborn is vulnerable

Examiner: "Why is this infant cold? Explain the physiology." [1]

  • The newborn loses heat through four pathways: evaporation (dominant at birth while wet), radiation (skin to cooler walls, the largest in a cold cot), convection (moving air), and conduction (cold surfaces). [1]
  • The newborn generates heat only by non-shivering thermogenesis in brown adipose tissue — shivering is poorly developed. [1]
  • The preterm is the extreme: enormous surface-area-to-mass ratio, high transepidermal water loss through immature skin, small brown-fat and glycogen stores (laid down late in the third trimester), and an immature hypothalamus. [3]
  • Thermogenesis consumes both oxygen and glucose; once glycogen is exhausted, hypoglycaemia deepens and thermogenesis fails — a vicious cycle. [9]
If the examiner asks about brown fat

Brown adipose tissue is concentrated around the neck, scapulae, axillae and along the great vessels, rich in mitochondria and uncoupling protein 1 (thermogenin). Sympathetic noradrenaline activates lipolysis; fatty acids are oxidised and the proton gradient is dissipated as heat. It is effective but finite — especially in the very preterm, whose stores are minimal. [1]
