Neonatal abstinence syndrome (NOWS) is the postnatal withdrawal syndrome that follows chronic in-utero opioid exposure, peaking at 24–72 hours. [3] Chronic exposure up-regulates μ-receptors and locus-coeruleus noradrenaline; cord clamping removes the opioid brake and unleashes the triad of CNS hyperexcitability, autonomic overdrive and gastrointestinal dysregulation. Methadone, with its long half-life, characteristically peaks at 48–72 hours, which is why this infant is declaring now.
