You are the sole doctor at a rural hospital with no on-site theatre, no anaesthetist and no blood bank. It is 01:15. A 26-year-old G2P1 at 37+4 weeks in spontaneous labour at 5 cm has just had a large spontaneous rupture of membranes. Her lie was reported as 'variable' at her last visit. The midwife tells you the fetal heart rate has been 60 to 70 bpm for 90 seconds. The nearest theatre is 55 minutes away by road ambulance; the retrieval service can be airborne in 40 minutes.

Station brief

Format. Drill-style emergency management station, 12 minutes, run against a clock the assessor calls aloud. You are the only doctor on site. A midwife and an enrolled nurse are with you. There is no theatre, no anaesthetist and no blood bank. The assessor plays the team, the ambulance service and the retrieval coordinator. [1][5]

Candidate instruction. Lead the management of a cord prolapse in a setting where you cannot deliver the baby yourself by caesarean. Verbalise every action, delegate by name and role, state volumes and doses, and be explicit about what you can and cannot achieve. You will be asked for your retrieval handover and your documentation. [1][3]
