Prompt
A 33-year-old woman at 17 weeks had amniocentesis for a fetal renal anomaly. The karyotype is normal and chromosomal microarray returns a variant of uncertain significance in a gene associated with a rare neurodevelopmental condition. She is distressed, asks whether the baby 'has it', and wants to know if she should terminate. You are the registrar; clinical genetics is available.

Station brief

Format. Communication station, approximately 12 minutes. You are the obstetric registrar. An actor plays the woman; the assessor feeds new information. Clinical genetics is available. [5]

Candidate instruction. Explain what a variant of uncertain significance is and is not, avoid recommending irreversible action on uncertainty, arrange clinical genetics referral and parental segregation testing, and support the woman through her distress. [5]
