You are the on-call gynaecology registrar at a tertiary fertility centre. A 31-year-old with PCOS and AMH 48 pmol/L presents 6 days after oocyte retrieval with tense abdominal distension, weight gain of 4 kg in 36 hours, vomiting, oliguria for 8 hours, BP 92/56 mmHg, HR 122, RR 26, SpO2 94 per cent, haematocrit 0.51. The trigger was hCG 10 000 IU on day of retrieval in an antagonist cycle with 28 follicles. The fertility consultant is at home and reachable by phone.

Station brief

Format. Emergency management station, approximately 12 minutes. You are the on-call gynaecology registrar. The fertility consultant is at home and reachable by phone. The assessor will run the clock and feed you new observations. [1]

Candidate instruction. Take a focused history, grade severity, lead the acute resuscitation bundle, defend the prophylaxis you would have used, manage complications (torsion, thromboembolism), and counsel the patient. Verbalise your actions, your drug orders with doses, your escalation triggers, and your communication with the team, the consultant and the patient.[1][2]
