Prompt
A hot case on the sedation choice — the midazolam stopped (the delirium, the accumulation), the dexmedetomidine started (the rousable, the delirium-sparing), the analgesia-first.
Presentation
You are at the bedside of a 70-year-old ventilated for 5 days on the midazolam 6 mg/h and the fentanyl 50 mcg/h. The CAM-ICU is positive and the RASS is -3. The SBT has failed (the rapid shallow breathing at 15 minutes). The nurse asks for the sedation adjustment.[1]
