Red flags
- Syncope WITH palpitations means an arrhythmic cause until proven otherwise — these patients need cardiology assessment, not reassurance
- A family history of sudden death under 40 raises inherited arrhythmia risk — refer even when the ECG is normal
- PVC burden above about 10% of daily beats risks PVC-induced cardiomyopathy — monitor, do not dismiss as benign ectopy
- Never label palpitations 'anxiety' before the ECG, TSH and full blood count are back
Overview
Palpitations are a common ambulatory complaint, and cardiac causes carry the most concern because sudden death is possible when structural disease or channelopathy sits underneath.[4] The consultation has three tasks: classify the rhythm by description, find reversible drivers, and match monitoring to how often symptoms occur.[4]
