A 15-year-old boy arrives with his mother: four hours of severe left testicular pain that woke him from sleep. He has vomited once. He is curled forward on the chair, pale. His mother says 'I think it's just a strain from footy.' No fever, no dysuria, no trauma he can recall.
Objectives
- Recognise probable torsion immediately and act on it — history and focused examination within minutes, not a full consultation. [4]
- Examine efficiently: swelling, hard testis, high-riding position, cremasteric reflex, vomiting. [4]
- Arrange emergency transfer for surgical exploration; do not order community imaging. [9]
- Communicate urgency to mother and boy without inducing panic; explain the time-sensitivity honestly. [7]
- Document onset time precisely — it drives salvage decisions downstream.
