A 42-year-old primary-school teacher attends requesting 'a pelvic scan and some tablets'. Periods have been heavy for three years — soaking protection hourly on days 2–4, clots the size of grapes, leaking at work twice monthly. Cycles regular. Two children; no plans for more. No intermenstrual bleeding, no pain. Looks tired; conjunctivae pale.
Objectives
- Diagnose HMB by its impact definition and elicit the structured history NICE specifies. [21]
- Exclude pregnancy and cervical pathology before any treatment pathway. [21]
- Order risk-appropriate investigations — FBC mandatory; imaging only where examination or history raises structural suspicion. [21]
- Offer LNG-IUS first-line with honest counselling about six-cycle assessment and bleeding-pattern change. [4] [21]
- Treat iron-deficiency anaemia alongside bleeding control and arrange tranexamic acid/NSAID cover meanwhile if needed. [21] [5] [6]
