Laryngospasm during inhalational induction
The hot laryngospasm crisis — the recognition (the crowing inspiratory sound, the tracheal tug, the paradoxical chest movement, the falling saturation), the escalation (jaw thrust, CPAP, 100% O2, suxamethonium if severe), and the prevention of the negative-pressure pulmonary oedema.

Presentation
During a sevoflurane inhalational induction of a 3-year-old, the child develops a bout of coughing, holds their breath, and the chest wall retracts with each inspiratory effort. The oxygen saturation falls rapidly from 98 to 82 per cent. There is no air movement despite the mask ventilation.
