The T-piece in paediatric anaesthesia
A focused case on the T-piece paediatric system — the valveless design (the low resistance), the high FGF requirement (2-3x minute ventilation), the Jackson-Rees modification (the reservoir bag for the manual ventilation), and the fresh gas flow calculation for a child.

Prompt
A 3-year-old child is being anaesthetised with a Jackson-Rees T-piece (Mapleson F) for a myringotomy. The fresh gas flow is set at 4 L/min. Discuss the adequacy of the flow and the principle of the system.
Presentation
A 3-year-old child (14 kg) is being anaesthetised with a Jackson-Rees T-piece (the Mapleson F) for a bilateral myringotomy tube insertion. The fresh gas flow is set at 4 L/min of sevoflurane in oxygen and nitrous oxide (50:50). The child is breathing spontaneously.
